yes it can, in order to be successful in receiving decent money through adsense, you need to drive a bunch of traffic to your site. for example, you make a blog post about a PTC site, you will post your referral link inside that post. if a visitor get interested with the content of that post, he/she will click for the link to register. The more visitors you have in your blog or site the chances of getting a lot of referrals. If they are satisfied with your offers, they will tend to come back to your site and wait for the next update you are going to make. It's like building a relationship with your visitors through quality posts.

How well can AdSense and referral advertising coexist on a site?

It seems to me it's easier to get paid by AdSense than by referral links, since it's harder for referrals to convert.
On the other hand, referral links will pay me much better in the long run if my site is an authority on the subject.

Can they co-exist successfully?

Referral Exchange is not a good way of promotion I think. This way you can find new referrals of course, but unfortunatly most of them are not active, so there is no point to have them right?

These are all great tips, though it is often a matter of trial and error before you find the best way to promote your individual link.
